The question "what does C+E actually cover" almost always means a different question: is what I already hold enough. Usually it is not, and it is better to find that out before agreeing to a trip.
Why C and C1E fall short
The categories differ by definition, not by feel — § 80a of the Czech road traffic act:

12 000 kg belongs to C1+E, not to C+E
The figure 12 000 kg circulates as "the C+E ceiling", and it is wrong by exactly one category. C+E is defined from below — a vehicle over 3 500 kg with a trailer over 750 kg — and carries no maximum combination weight at all. If it did, an ordinary international rig would not fit inside the category it is driven on.
Note too that 750 kg does two opposite jobs in the same section: for bare category C it is the ceiling on what you may tow, while for C+E it is the floor at which a combination enters the category.
On top of that, § 81 odst. 1 písm. f) says C+E "opravňuje také k řízení vozidel zařazených do skupin B+E a C1+E" — hold C+E and the lower combination categories need not be taken separately.
From what age
C and C+E are granted to someone who has reached 21 — § 83 odst. 1 písm. e): "Řidičské oprávnění lze udělit jen osobě, která dosáhla věku […] e) 21 let, jedná-li se o skupiny C, C+E, D1, D1+E".
There is a real exception, and it matters in this trade: under § 83 odst. 5, C and C+E may also be granted from 18, but only in connection with initial professional training (vstupní školení), and the entitlement stays restricted until 21. So it is not "eighteen is enough" but "eighteen plus the initial training, and with a restriction".

How it is obtained
C+E comes after C: the vehicle first, then the combination. Training is theory plus practical driving on an actual combination — reversing with a semi-trailer, manoeuvring, coupling and uncoupling. This is not the part anyone passes first time over a weekend; budget weeks rather than days.
No statute fixes what a driving school charges or how many hours it runs, so no figure is given here: it varies between schools and between countries, and a single rate simply does not exist. What is fixed you can read for yourself — the age limits and category definitions are in Act No. 361/2000 Coll. (§§ 80a, 81 and 83), the scope of professional training in Act No. 247/2000 Coll., § 48. For the rest, ask a specific school, and ask separately about combination driving time, because that is usually what decides the total.
Code 95: how many hours and how often
Professional competence is a repeating cycle, not a one-off exam. Periodic training runs to 35 hours by the end of the fifth year from the date the card was issued, split into annual courses of 7 hours; an annual course may be spread over two consecutive days.

A driver coming back after a break, whose cycle has lapsed, does all 35 hours in one go under § 48 odst. 4. That is the reason not to let an annual course slide: 7 hours once a year is a single day, a lapsed cycle is a whole week.
The order to do three things in
An international driver needs three documents, and the order saves weeks:
- Category C+E — the longest part, so start here.
- Code 95 — professional competence, 35 hours over five years at 7 hours a year. It can run alongside the practical driving.
- The tachograph card — apply straight after C+E: the authority has up to 1 month from a complete application, and it is the easiest of the three to forget. The procedure is in The tachograph card.
